To receive your message digitally your healthcare provider will need to have registered at this website.

If your healthcare provider has not yet registered at this website, you can arrange your permission here by filling in a form, printing it out and giving it to your healthcare provider.

If your healthcare provider has deregistered from this website, you cannot arrange your permission via this website. In that case, please contact your healthcare provider.

Once you have selected your healthcare provider you will be able to find out whether you can use DigiD with SMS and whether you can arrange your permission via this website.

Bear in mind that your log in session with DigiD expires after 15 minutes inactivity. You will then need to log in again.

On this website you can give or withdraw your permission for your medical information to be made available for viewing via the LSP.

If you give permission, your GP or pharmacy will note that in your medical record. Your most important medical information will then be available to other healthcare providers. They include a stand-in GP, pharmacist or medical specialist at the hospital. They are only permitted to view your details if necessary for your treatment.

It always remains possible to withdraw your permission. Without your permission other healthcare providers are not permitted to view your details via the healthcare infrastructure. Not even in emergencies.

What do you have to do?

    Select No. It is always possible to withdraw your permission. It also remains possible at all times to indicate that you want to give permission.

    What should you do if you don't want your details to be shared via the LSP?

    You do not need to do anything. Your healthcare provider is only allowed to make your details available to other healthcare providers with your permission.

    If you have lodged an objection, there is nothing you need to do. Your medical information cannot be viewed via the LSP.

Important information!
